We are currently accepting adult only new patients by referral only. The referral can come from your current primary care doctor (PCP), counselor (LPC), or any other treating provider. Your provider can either send us secure fax with a referral to 833-553-2040 or fill out a professional secure referral e-form on our website. We will contact you directly once we are able to review the referral form.
